Upload the Forms That Users Will Review

The Forms template allows you to provide the metadata for the booklets that will be reviewed in a session, specifying which item assertions appear in the booklet and what context values will be used for the booklet.

Sheet Column Description
Forms Form

Name of the test form as it will appear in the Standard Setting Tool.
Forms Description Description of the corresponding form.
Forms MarkSet

Proficiency level reference set that will be used in the standard setting session. Must be consistent with the Markset field across all template sheets.
Forms Grade Grade level associated with the test form. This field also controls the order that tests/forms appear in the Psychometrician view and will recognize 0 if needed for kindergarten tests.
Forms Subject Subject associated with the test form. This groups Forms by subject in the Psychometrician view and allows for that user to toggle between Confirmed Line graphs.
Forms TestKey Unique test key for the booklet.
Forms IsDemo

Requires all Panelist marks be set if that particular step is a mark-setting step.

  • Set to TRUE for all test experience steps
  • Often set to TRUE for practice OSAB steps (depending on whether they want an additional practice on room lead validation)
  • Always set to FALSE for regular OSAB to ensure that all users in the round are confirmed/validated before the Room Lead can change steps

FormItems Form

Name of the test form as it will appear in the Standard Setting Tool.
FormItems BankKey

ITS bank ID. Should match ITS bank used for content package extract. Must be consistent with BankKey field across all template sheets.
FormItems ItemID

This is the ID for the item in a particular form. It will appear multiple times for 2+ point items in the OSAB form.
FormItems Position

Sequence in which the item appears in the test form or booklet.
FormImpacts Form

Name of the test form as it will appear in the Standard Setting Tool.

Must match the name of a Form on the Forms sheet.
FormImpacts Impact

The impact measure used for this form.

Must match an impact uploaded in the Basics template.
FormImpacts SortOrder

Sequence in which the item appears in the test form or booklet.
AssertionImpactValues Form

Name of the test form as it will appear in the Standard Setting Tool.

Must match the name of a Form on the Forms sheet.
AssertionImpactValues BankKey

ITS bank ID. Should match ITS bank used for content package extract. Must be consistent with BankKey field across all template sheets.
AssertionImpactValues ItemID Unique ID for the item as it appears in ITS.
AssertionImpactValues AssertionId

ID for the assertion within the item.
AssertionImpactValues Impact

The impact measure used for this assertion.

Must match an impact uploaded in the Basics template.
